New Patient Paperwork
Please follow the link to our electronic new patient paperwork. When you complete the paperwork electronically, it will automatically be sent to our office. Please do not print the forms, only complete them online. If you have any questions or concerns, please give us a call. Patients are also welcome to complete the electronic paperwork in office, but please arrive 20 minutes prior to your scheduled appointment time.

Dental Insurance
We gladly accept all PPO dental insurance plans. All PPO insurances will allow you to use out of network benefits at our office. We know insurance can be complicated and confusing, so please call our office and speak to our office manager if you haveany questions.
As a courtesy to our patients, we submit dental claims for all services rendered. Payment is due when services are rendered unless prior arrangements have been made with our office. Please bring your insurance card and insurance plan information with you to your New Patient Appointment.
